New Student Registration Form
Available to students entering grades 1-12.- Please fill out the New Student Registration Form about your student:
- 2025-2026 Form - Entering the district beginning Fall 2025
- The following items will be needed upon registration:
- Birth Certificate (Hospital records will not be accepted)
- Social Security Card
- Immunization Record
- Insurance/Medicaid Card

After completing the forms, please return them to your child's campus. Forms may be completed at their campus.
*Home Language Survey is available in Arabic, Chinese, Hmong, Laotian, Marshallese, Tamil, Telugu, and Vietnamese upon request. -
